EPS

Our pharmacy offers the Electronic Prescription Service, which allows you to choose or 'nominate' a pharmacy to get your medicines from. Your GP then sends your prescription electronically to your nominated location.


The Electronic Prescription Service (EPS) allows prescribers to send prescriptions electronically to a dispenser, such as a pharmacy, nominated by the patient. This makes the prescribing and dispensing process more efficient and convenient for patients and healthcare workers.

If you collect your repeat prescriptions from your GP you will not have to visit your GP Practice to pick up your paper prescription, saving you time.
 

Please complete and submit this form to The Revel Pharmacy as an EPS notification. The form is available in PDF format and can be either emailed to pharmacy.fph05@nhs.net or printed and delivered directly to the pharmacy.

The Revel Pharmacy will process and submit this on your behalf.
